Job Description

LPC wanted for integrative clinic! Willing to consider an associate. Well Life Medicine is an integrative physical and mental health clinic with locations in Salem and Hillsboro. We offer a unique integrative medical model with primary care providers, and mental health specialists. We offer telehealth visits as well as in-person visits. Strong preference for hybrid, may consider remote only. This is a salaried position. FT or PT available.
Job Overview:
The counselor will be responsible for direct patient care; intakes, warm hand-offs from primary care and psychiatric mental health providers, and follow up appointments. The ideal candidate for this position is patient-oriented, team player and enthusiastic about mental health care. Candidates must be currently licensed in Oregon. The Mental Health Counselor will work as a member of a multi-disciplinary team providing counseling, case management and integrated dual diagnosis treatment. The team works to address all treatment needs and collaborates with other healthcare providers at Well Life. Hours/days flexible. This can be all ages or adult-only.
Duties:
  • Individual Counseling:
    Provide trauma-informed, individual counseling to clients regarding mental health symptom management, substance use, personal and situational stressors, relationships, suicide and other risk concerns utilizing psychoeducational and evidence-based counseling practices.
  • Mental Health Assessment and Care Planning:
    Collect information regarding clients' history through a combination of interviewing, records review, and consultation, after obtaining written consent.
  • Conduct and document mental health assessments, including mental status exam, DSM diagnosis, comprehensive risk assessment, and clinical formulation.
  • Develop care plan with client: monitor client's progress and adjust their care plan as needed.
  • Risk Assessment and evaluation: Provide crisis intervention which may include: counseling, supporting client with de-escalating, performing risk assessments, supporting with safety planning, and referring clients to the ED or other sub-acute facilities.
  • Participate in outreach activities necessary to offer services and engage participation in a noncoercive manner.
  • Maintain client records using electronic medical records.
Must be documented as required by program standards and funding requirements including documentation of clinical interactions, behavioral health assessments & individual service plans.
Requirements:
  • A minimum of a master's degree in counseling, social work, or a related field
  • Licensure in Oregon in chosen discipline
  • A familiarity with, and adherence to, the ethical code of the American Counseling Association Please send a cover letter and your resume.
