Job Description

Job Title:
Wellness Specialist Job Summary:
CASES is seeking a Wellness Specialist to join the FACT team. This is a Masters level mental health clinician who outreaches clients in the community to provide treatment and services with high fidelity to the ACT model. The Wellness Specialist provides engagement services, clinical assessment, and psychoeducation to clients, utilizing the Wellness Recovery Action Plan (WRAP) when clinically appropriate, with the overarching goal of promoting recovery and stability in the community. Specific title does not limit the scope of clinical services that the staff member provides as FACT staff are both generalists and specialists, based on their areas of experience and interest.

The purpose of Forensic Assertive Community Treatment (FACT) is to deliver comprehensive and effective services to individuals (age 18 and up) who are diagnosed with severe mental illness (SMI), suffer from co-occurring substance use disorders, have histories of criminal legal system involvement, and whose needs have not been well met by more traditional service delivery approaches. Assertive Community Treatment (ACT) is an evidence-based practice that provides an integrated set of other evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multi-disciplinary mental health treatment team to support recipient recovery through a highly individualized approach that provides recipients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects.
Salary:
The salary range for the Wellness Specialist is $68,959 (LMSW/MHC-LP) to $72,000 (LCSW/LMHC) per year.
Shift Hours:
The Wellness Specialist position is full-time working Monday through Friday from 9:00 am to 5:00 pm.

Workplace Flexibility:
Flexible (for roles that have days that can be partially remote, e.g., being able to draft notes at home after going to the office/community in the morning). Staff are able to start their day remotely 1 day per week (Mondays).
What you will be doing:
Provide 80 percent of treatment contacts in the community and complete expected minimum monthly treatment contacts, including leading groups when clinically indicatedAttend and participate in daily team meetings to discuss clients clinical and case management issues and needsComplete documentations required by the NYS Office of Mental Health (OMH) and NYC Department of Health and Mental Hygienes (DOHMH) licensing guidelines, NYS Medicaid billing guidelines as well as per

Policy and ProcedureCollaborate with clients support network and other service providers on treatment planning, service coordination, and case management support with the goal of promoting clients recovery and stability in the communityProvide clinical services that are anti-racist, gender-responsive, culturally humble, trauma-informed, recovery-oriented, client-centered, and evidence-basedComplete required documentations and toxicology tests for clients with active Assisted Outpatient Treatment (AOT) orderProvide 24-hour crisis intervention on-call services on rotating basisProvide culturally competent services in accordance with CASES policies and practice What we are looking for: LMHC, MHC-LP, LMSW, LCSW, with current New York State licenseExperience with various evidence-based models pertaining to treatment of substance use, and mental illnessA team player, highly organized and self-motivatedProficient in computersIndividuals with lived experience are encouraged to apply
