We provide compassionate, culturally responsive, whole-person care through an integrated model that blends primary care, counseling, chronic care management, and wellness services. We are expanding our behavioral health program and seeking an LPC who values equity, collaboration, and patient-centered care. Position Overview Legacy Health is seeking a Licensed Professional Counselor (LPC) to deliver outpatient counseling services to adult patients within a supportive, integrated medical setting. This role is ideal for a clinician seeking autonomy, a healthy workload, and a team that truly values mental health. Responsibilities Provide individual counseling for adults experiencing anxiety, depression, trauma, grief, stress, adjustment-related concerns, and those seeking weight loss services. Complete biopsychosocial assessments and develop evidence-based treatment plans. Deliver trauma-informed, culturally responsive, strengths-based care. Collaborate with physicians and care team members as part of our integrated Behavioral Health Integration (BHI) model. Participate in warm handoffs, care coordination, and shared treatment planning, as indicated. Maintain accurate and timely documentation in the HER (eClinicalWorks). Provide crisis intervention and safety planning when necessary. Refer patients to community resources and coordinate follow-up care. Participate in team meetings and quality improvement initiatives. Qualifications Masters degree in counseling, Clinical Mental Health Counseling, Psychology, or related field. Active Texas LPC license Experience providing outpatient counseling to adults preferred. Knowledge of integrated care or primary care collaboration is a plus. Strong communication skills and commitment to equitable, patient-centered care. Comfortable working independently and within a team. Proficient with EHR documentation. Proficiency in Spanish is a plus, but not required.
