As a Social Worker (LMSW, LCSW, LPC) on our team, you will play a crucial role in delivering compassionate and individualized care to our patients and their families. About the Role We are seeking a highly motivated and experienced Social Worker to join our interdisciplinary care team. As a Social Worker, you will work closely with our multidisciplinary team to assess patient needs, develop individualized care plans, and connect patients and their families with community resources. Your expertise will help us provide comprehensive care that addresses the physical, emotional, and social needs of our patients. Responsibilities Conduct initial assessments and ongoing evaluations to identify patients' social, emotional, and psychological needs Develop and implement individualized care plans that address patients' needs and goals Collaborate with patients, families, and healthcare providers to ensure that patients receive seamless and coordinated care Connect patients and their families with community resources, including social services, support groups, and counseling Provide emotional support and counseling to patients and their families Responsible for delivering and supervising group therapy sessions on designated units, collaborating with multidisciplinary teams to assess needs, ensure safety, and promote recovery, engagement, and ethical practice.
