Job Description
About True Life Destinations (TLD): TLD is a licensed Department of Behavioral Health & Developmental Services (DBHDS) provider looking for qualified individuals to work in the human services field. TLD provides Mental Health Skill-Building Services (MHSS) to adults (18 and above) in their home and community environments.
- TLD is looking for Full Time employees to serve individuals in the Mental Health Skill-Building Services Program.
NO PART TIME OPPORTUNITIES AVAILABLE IN THIS POSITION
- TLD is specifically looking for individuals to be housed on 2 different teams who can work in Hampton/NN/Yorktown/Williamsburg areas, and then the Southside areas (Portsmouth, Norfolk, Chesapeake, and Virginia Beach areas)
- TLD is also specifically looking to hire males to work male caseloads (both sides of water)
- TLD's office is located in Hampton, Virginia but serves all of Hampton Roads. Flexible scheduling allows you to work with clients in their home and community environments.
- TLD is allowed by DMAS regulations to offer Telehealth opportunities to clientele.
Required Qualifications:
Applicants must have a minimum of 1 year of experience working with the adult mentally ill population OR Have a degree in the field (social work, psychology, human services, etc) and qualify to apply for QMHP trainee licensure through the Board of Counseling if no experience. Must be willing to obtain supervision through this agency if so. OR Have a degree in the field and experience with adults with mental ilness. Prefer QMHP or QMHP trainee license already in place if possible OR to be eligible to apply and receive license quickly. Applicants must be able to provide excellent interpersonal skills, be knowledgeable about the mentally ill population, demonstrate great documentation skills and be able to develop strong positive connections with other community agencies. Applicants with Vocational experience, Social Service background, Community Service Board background and Probation Parole experience is preferred but not required. Responsibilities Assess and monitor adults with serious mental illness (SMI) in various settings, focusing on ADL instruction, daily living skills, and independence goals. Conduct psychoeducation and skill-building in personal care, hygiene, nutrition, money management, and community integration. Develop individualized care plans, track progress in medical and behavioral domains, and coordinate with healthcare teams and community resources. Facilitate telehealth or in-person sessions, documenting assessments and interventions while maintaining HIPAA compliance. Provide discharge planning support and caregiver education to promote lasting daily functioning and safety.Job Type:
