Job Description

(QMHP-/Trainee) (MHS)
Position Description:
Under the supervision of the clinical director, the QMHP Workers will provide direct QMHP services that implement a service plan, which provides structure, support, and training and support to enable individuals with significant psychiatric functional limitations to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. QMHP may be provided in any setting in which the individual's needs may be addressed, skills training applied, and recovery experienced.
Qualifications:
Completion of a Bachelor's Degree in social work, psychology, or other DMAS approved human service field and with at least one year of clinical experience providing direct services (developing, conducting, and approving assessments and individual service plans or treatment plans) to persons with a diagnosis of mental illness. A valid Virginia Driver's License is also required for employment. Must be registered with the Department of Virginia Board of Counseling as a QMHP-A and provide copy of registration in compliance with changes that went into effect (January 1,2018).
Duties and Responsibilities:
Conduct intake and orientation sessions with clients Develop and implement individualized service plans with client and/or parent/guardian in coordination with the Program Clinician and referral agency. Develop and maintain a network of supportive community referrals for clients Refer and link clients to community resources Monitor and ensure the effectiveness of referrals Conduct monthly and quarterly assessments with clients to evaluate progress and needs. Establish working relationships with representatives of other agencies who work with and provide services to clients. Provide care coordination and casework services to clients Provide training and experiential learning activities to clients as outlined in the client's individual service plan in daily living skills of menu planning, grocery shopping and meal preparation, job search and maintenance, money management and banking, community resources, health and safety, legal issues, housing search and maintenance. Provide transportation to clients as needed. Provide 24 hour on-call coverage to clients on their caseload Maintain, update and review case records Ensure that all services and casework documentation is submitted in a timely and meets or exceeds licensing, Medicaid standards and Hope Seekers Community Support Services Report on statistical and utilization data and service outcomes. Models and teaches problem-solving techniques. Coordinates scheduling of appointments, as needed. Coordinates and provides transportation, as needed. Maintains regular and ongoing contact with referring agencies, as well as other agencies involved in youth and family's case. Actively coordinates and plans discharge and aftercare plans with referring agency. Attends interagency meetings, as necessary. Attends and participates in team meetings. Attends and participates in orientation, in-service training, and conferences as scheduled. Attends supervision. Performs other duties as assigned. Attend and participate in new employee orientation, in-service training, conferences as scheduled and performs other duties as assigned. Receive 40 hours of training per year, maintain and keep current all first aid and CPR training requirements. I understand that by taking this position I am required to be drug free and will not be intoxicated while at work. I understand that I may be drug tested at anytime.
