Job Description

Overview We are seeking a dedicated and compassionate Qualified Professional to join our team. This role is essential in providing high-quality care and support to individuals facing mental health challenges. The ideal candidate will possess a strong background in mental health skill building, crisis residential, and 23-Hour/ community stablization demonstrating a commitment to fostering positive outcomes for clients through effective therapeutic interventions. Duties Responds to referral source and family in need of services within 1 hour of receiving referral from clinical supervisor or providing up to 23-hour supervision and services to an individual within the receiving center. Provides services on a short-term, solution focused treatment strategies to clients and families. Facilitates care coordination with other service providers to enlist existing supports in determining and meeting service objectives and to identify additional resources and support. Provides direct clinical services to clients, including individual and family interventions. Responds to crisis situations with twenty-four hour a day availability. Completes progress reports in reference to service delivery weekly. Utilizes Noteable (Electronic Medical Record system) and other web-based software for all clinical documentation, record keeping, and coordination. Pass and complete DBHDS, DMAS, and

required trainings on an ongoing annual basis to include but not limited to: HIPAA, Bloodborne Pathogens, MANDT, CPR-First Aid, Person Centered Practices, Human Rights, Mandated Reporting, and Emergency Preparedness. Links clients with external programs or services, such as: health services, recreational activities, child -care services, financial resources, housing placement, employment resources, AA/NA groups, transportation resources, and others. Attends court hearings and probationary meetings when families are court-involved and presents testimony and treatment recommendations as requested by attorneys, referral source, the court, or clients. Meets Clinical Supervisor weekly (minimum) for service delivery and collateral contact review, chart audits, case staffing's, individual training, self-care check-in, and employee development planning. Attends Monthly staff meetings, group supervisions, and/or trainings. Prepares written Discharge Summary and Plan. Adheres to all company policies and procedures while completing other task deemed necessary for the successful and quality-driven delivery of services. Maintain an accurate schedule of individual sessions to be reviewed at any time. Documentation of all clinical documents required by state and licensure regulations; to reflect clients' progress towards completion of service goals and objectives.
Education & Experience:
A bachelor's degree in social work, Psychology, Counseling, or a related field from an accredited college or university and one year of experience providing mental health treatment to adults and families. Preferred master's degree in social work, Psychology, Counseling or related human services field from an accredited college or university with one year of experience in the mental health field working with adults. The successful candidate must be trained and experienced in providing mental health services to individuals who have a mental health illness. Candidate must meet all qualifications for QMHP-A/T or LMHP-R and be in good standing with the Virginia Board of Health Professionals.
