Job Description

Job Opportunity:
Mental Health Support Specialist Hiring multiple positions in both Winchester, VA and Harrisonburg, VA Connected Communities is seeking a Mental Health Support Specialist to provide mentoring, skill-building, and individualized support to youth, adults, and families in home and community settings. In this role, you'll help individuals build life skills, overcome challenges, and achieve personal goals through meaningful, relationship-based work. HIGHLY Preferred Essential Functions Direct Service Delivery:
Provide one-to-one support services to youth, adults, and families in home and community settings Engage referred individuals by initiating contact and scheduling sessions at mutually agreed-upon locations Establish positive, respectful, and supportive relationships that foster trust and engagement Deliver structured, outcomes-focused sessions that support personal growth and development Adapt approaches to meet the unique needs, abilities, and emotional states of each individual
Skill Development & Mentoring:
Support individuals in developing life skills, including social, emotional, and independent living skills Assist individuals in setting and working toward personal, educational, and behavioral goals Utilize mentoring techniques to provide guidance, encouragement, and constructive feedback Integrate activities that promote personal growth and skill acquisition
Care Coordination & Resource Linkage:
Provide education and awareness of available community, educational, and economic resources Assist individuals and families in accessing appropriate services and supports Communicate regularly with case managers and other service providers to ensure coordinated care Develop and maintain relationships with schools, community organizations, and other partners
Documentation & Reporting:
Monitor individual progress and document services following each session Complete required reports and submit documentation in a timely manner Gather and track outcome and evaluation data to support program effectiveness Maintain accurate and organized records in accordance with organizational standards
Qualifications Required:
High school diploma or equivalent (Bachelor's degree in a human services field preferred) Experience working with youth, families, or adults in a mentoring, coaching, or support role Experience working with individuals facing social, emotional, or educational challenges Ability to build rapport and maintain supportive, professional relationships Strong communication, organizational, and time management skills Ability to work independently in community-based settings Demonstrated empathy, professionalism, and adaptability Ability to maintain confidentiality in accordance with organizational policies Physical & Work Requirements The physical requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of this position, with or without reasonable accommodation. This position operates in home and community-based environments and requires: Frequent standing, walking, and travel within the community Prolonged sitting during sessions or documentation Bending, reaching, and light lifting (up to 25 pounds) Clear verbal communication with individuals, families, and service providers Safely entering and exiting vehicles The employee must: Possess a valid driver's license and maintain an acceptable driving record Be able to safely operate a motor vehicle Sit for extended periods while driving The noise level may range from moderate to high depending on the service environment. $17.00 - $24.00 per hour
