Outpatient Therapist/FT/West Bend | Miramont Behavioral Health | Waukesha, Wisconsin About the
Job:
PURPOSE STATEMENT
The Substance Abuse Counselor provides treatment and support to assist patients in their recovery from addictions, such as alcoholism, drug addiction, assists with modifying problem behaviors. Responsibilities include screening clients for substance use disorder issues, performing assessment, diagnostic evaluation, crisis intervention, case management, and therapeutic services for patients and families. Conducts individual and group sessions to assist with management of symptomology and coping strategies. Evaluates patients progress during Counseling and will work collaboratively with a multidisciplinary team in achieving an optimal quality of life.
Roles and Responsibilities:
ESSENTIAL FUNCTIONS

  • Works within a multidisciplinary team to formulate and prepare personalized treatment plans to fit individual needs.
  • Facilitates both process and psychoeducational groups, individual sessions, and family sessions with individualized care considerations.
  • Prescribes a continuation of Treatment plan to support ongoing stabilization and recovery efforts.
  • Conducts screenings, assessments, referrals and follow-ups for patients.
  • Provides case management duties for patient, ensuring individualized quality care.
  • Evaluates the needs of the client and determine if referrals to other programs or facilities is needed.
  • Ensures all documentation regarding patient care, treatment, and incidents are completed timely and in a clear concise manner.
  • Provides crisis intervention to patients as needed.
  • Plans for aftercare for assigned patients.
  • Acts as a liaison with insurance carriers as requested.
  • Establishes and maintains effective working relationships within the agency, community and referral sources.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS
  • High School diploma or equivalent required. Bachelor's degree in social or health services field preferred or as required by the state in which the facility operates.
  • Two or more years' experience in providing substance use disorder interventions and case management with knowledge of addiction recovery dynamics required.
LICENSES/DESIGNATIONS/CERTIFICATIONS
  • Licensure or certification for counseling, as required by the state in which the facility operates;
  • CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
  • First aid may be required based on state or facility.
