Case Manager SMI (Behavioral Health)
Starting Pay:
$22.00 per hour

Cottonwood, AZ Position Summary The Case Manager provides comprehensive care coordination and support services to individuals receiving behavioral health treatment. Working under professional supervision, the Case Manager helps clients access resources, navigate healthcare systems, achieve treatment goals, and improve overall health and wellness outcomes. This role serves as an important link between clients, providers, and community resources by coordinating care, supporting treatment plan implementation, and advocating for client needs. The Case Manager works collaboratively with multidisciplinary teams to promote recovery, stability, independence, and improved quality of life. As integrated healthcare continues to evolve, Case Managers are expected to develop expertise in behavioral health interventions, healthcare navigation, care coordination, and the management of common physical and behavioral health conditions affecting the populations served. Key Responsibilities Coordinate care and treatment activities for assigned clients. Participate in client staffing and implement treatment plans. Provide supportive counseling and document services using the DIAP format. Conduct assessments and reassessments under supervision. Develop and update treatment plans in compliance with applicable guidelines. Collaborate with internal teams, healthcare providers, and community agencies. Maintain accurate and timely client records. Support clients in navigating healthcare systems and accessing services. Ensure compliance with all federal, state, and organizational standards. Maintain clinical productivity and participate in team meetings. Demonstrate professionalism, cultural competence, and ethical standards. Education & Experience Requirements Candidates must meet one of the following qualifications: High school diploma or GED and two (2) years of behavioral health experience. Bachelor's degree in any field and one (1) year of behavioral health experience. Bachelor's degree in a human services-related field. Completion of a Behavioral Health Technician (BHT) training program. Master's degree in a human services-related field (non-licensed). Preferred Qualifications At least one (1) year of behavioral health experience.
For High Needs Case Managers:
Two (2) years of case management experience and a relevant bachelor's degree (or an equivalent combination of education and experience). Skills & Qualifications Knowledge of behavioral health and case management practices. Understanding of common health conditions and wellness strategies. Ability to effectively navigate healthcare systems. Strong crisis assessment and intervention skills. Working knowledge of DSM-5 diagnostic criteria. Strong documentation and communication skills. Ability to work collaboratively in a team environment. Basic Requirements Must be at least 18 years of age. Valid driver's license and acceptable driving record. Ability to obtain a Level 1 Fingerprint Clearance Card. Current CPR/First Aid certification, or the ability to obtain certification within six (6) months of hire. Basic computer skills and the ability to utilize electronic documentation systems. Ability to work flexible hours as needed. Work Environment & Physical Requirements Frequent sitting, standing, and movement within a clinic or community-based setting. Occasional lifting of up to 15 pounds. Ability to work under pressure and manage competing priorities. Frequent communication in person, by phone, and electronically. #Spectrum
