Job Description

SMI Case Manager Horizon Health & Wellness Florence, AZ 85132 Posted about 3 hours New The SMI Care Managers main purpose is to ensure their client caseload is provided with the necessary services they need, in a coordinated and effective manner. Responsible for the caring and compassionate delivery of case management, advocacy and other supportive services to ensure appropriate coordination of services for clients.
Qualifications Required:
HS diploma or GED plus 4 years in Behavioral Health or combined Behavioral Health education and experience to meet Behavioral Health Technician qualifications. One year case management experience strongly preferred. Ability to create plans and summaries based on client needs Certification in first aid, CPR and Crisis De-escalation (training will be offered if do not possess upon hire). Must be at least 21 years of age or older. Possess valid drivers license with acceptable driving record and reliable transportation. Prior experience in the behavioral health setting is strongly preferred. Demonstrated organizational abilities to maintain communication, coordination, and connections with clinical team participants. Willingness to work evenings as needed. Desire and capacity to maintain time-management and organization while working in fast-paced, hands-on work environment. Must obtain and maintain a fingerprint clearance card and pass a pre-employment drug and background check. Have a Tuberculosis screening test and provide evidence of freedom from infectious tuberculosis prior to providing services and additional TB testing or screening questionnaires will be performed annually. CPR/First Aid Certification, training will be provided if the candidate does not possess active certification. Possess the physical, mental, and cognitive ability to perform specific clinical and/or tasks required by the position without posing a direct threat to the safety and security of patients. Mental and Emotional Ability to prioritize workload. Sound judgment and decision-making ability Ability to maintain calm, non-defensive, supportive attitude during difficult interactions. Demonstrate excellent verbal communication skills. Ability to analyze variables and situations. Ability to resolve problems, handle conflict and make effective decisions under pressure. Ability to think on his/her feet in the public spotlight. Physical Long periods of sitting, ability to bend, lift and carry up to 50 pounds. Able to operate phone, Fax, copier, and a computer. Able to operate Agency vehicle. Qualifications Preferred Bilingual - English/Spanish Experience working in Integrated Healthcare, Behavioral Health, or Primary Care environment.
