Job Description

Job Summary We are seeking a dynamic and compassionate Behavioral Case Manager to join our multidisciplinary healthcare team. In this vital role, you will coordinate and deliver comprehensive case management services to patients with complex behavioral health needs, ensuring seamless integration of mental health, addiction counseling, and medical care. Your expertise will help optimize patient outcomes by facilitating appropriate treatment plans, discharge planning, and post-acute care coordination. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations across inpatient, outpatient, and community settings. Duties Conduct thorough patient assessments focusing on behavioral health, mental health, addiction issues, and physiological factors to develop individualized care plans. Collaborate with hospital teams—including ICU, emergency medicine, pediatrics, and trauma centers—to coordinate inpatient and outpatient services tailored to each patient's needs. Manage utilization review and medical records documentation in compliance with HIPAA regulations to ensure accurate, timely information sharing. Facilitate discharge planning by coordinating post-acute care services, outpatient clinics, and community resources to support ongoing recovery. Perform diagnostic evaluations and manage case loads involving patients from Level I Trauma Centers, Level II trauma centers, PICUs (Pediatric Intensive Care Units), and other acute care settings. Provide addiction counseling support and behavioral health interventions to promote recovery and stability. Engage in patient service activities such as intake assessments, patient education, and ongoing monitoring of treatment progress. Maintain up-to-date knowledge of medical terminology, physiology, hospital medicine practices, and managed care policies to deliver expert case management. Support utilization management efforts by evaluating patient eligibility for services and ensuring appropriate resource utilization across inpatient and outpatient environments. Experience Proven experience working within hospital settings such as ICU, emergency departments, or trauma centers—preferably at Level I or Level II Trauma Centers. Extensive background in behavioral health case management within acute care or post-acute care environments. Hands-on experience with hospital medicine workflows including discharge planning, medical records management, intake procedures, and diagnostic evaluations. Strong familiarity with mental health counseling techniques, addiction treatment modalities, and social work practices. Knowledge of HIPAA regulations combined with excellent patient assessment skills to ensure privacy while delivering personalized care. Prior involvement in managed care programs or utilization management processes is highly desirable. Experience working with pediatric populations in PICU or outpatient clinics is a plus. Ability to navigate complex healthcare systems involving inpatient units, outpatient clinics, and community-based services effectively.
