Full Job Description Summary of Position:
The Case manager Specialty Behavioral Health provides high quality patient-centered care coordination as an integral part of a dynamic multi-interdisciplinary team. Care is provided in the clinic and outside in the community meeting each of our patients "where they are".
Essential duties:
Duties include, but are not limited to: Attends team huddles when in clinic, clinic staff meetings, and team meetings. Conducts patient consultations, assessments, and care planning in collaboration with healthcare professionals. Educates patients on conditions, treatment options, and available resources. Coordinates services, referrals, and transitions of care, including housing, employment, and crisis interventions. Monitors patient progress, responding to changing needs, and facilitating re-engagement using trauma-informed approaches. Advocates for patients and ensures compliance with healthcare regulations and confidentiality standards. Supports hospital discharge planning and follow-up to reduce readmissions. Completes documentation in the electronic medical record and manages prior authorizations. Serves as a point of contact for patients and advocates to resolve service-related issues. Performing other duties as assigned
Qualifications and Education:
Basic Knowledge and Skills:
High school diploma or GED work experience. Required to maintain a valid Arizona driver's license and pass motor vehicle screening. BLS Certification Fingerprint clearance issued by Arizona Department of Public Safety required. Experience in working with underserved populations is highly desirable. Must be at least 21 years of age.
Experience:
Minimum of 4 years' experience of full time behavioral/integrated healthcare experience.
US Skills:
Organization and care coordination Communication and teamwork Proficient with HMIS. Crisis intervention and de-escalation skills Advocacy for client needs and service access Knowledge of community resources and systems navigation, employment resources and Vocational Rehabilitation referral processes. Knowledge of substance use and/or mental health disorders Trauma-informed care
