Duties Include:
Conducts assessments or screenings with clients to evaluate the nature of their presenting problems in order to determine their needs and develop appropriate treatment plan goals. Develops and provides individual treatment plans; monitors, evaluates and documents client's progress. May have responsibility for intake documentation. Prepares progress notes, event notifications and other documentation regarding clients in accordance with regulatory requirements and company policies and procedures. Monitors case load daily to ensure required documentation is completed timely; including, but not limited to, assessments, treatment plans, case reviews and discharge plans. Provides direct support services to include engaging clients in Independent Living Skills (ILS) activities, health and wellness promotion, personal assistance services, rehabilitation and vocational skills, training and development services, transportation services and crisis management. Facilitates coordination of care to include case review, therapeutic interventions, and client progress in treatment. Participates in clinical staff meetings, trainings and other activities as required. Completes administrative tasks which may include, work orders, copying and faxing. Benefits Multiple medical plans

e. MA, MC, MSW, Arizona behavioral health license (other than LBSW) or eligible for Arizona license from Arizona Board of Behavioral Health Examiners. Must pass background check in accordance with Terros Health and DCS requirements including, but not limited to, no restraining orders within the last 10 years. Must pass DCS Central Registry check without any disqualifying events. Must have ability to perform First Aid or CPR as necessary and appropriate. Knowledge in use of Electronic Health Records
  • NextGen a plus. Experience working with diverse populations and bilingual desired (Spanish/ASL
  • pay differential available).
Proficient in Microsoft Word, Excel, PowerPoint, Outlook. Excellent oral and written communication skills. Minimum typing speed of 25 wpm. Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health's driving policy. Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role. Must pass a TB Test.
