Full Job Description Position Summary:
We are seeking a dedicated Intake Case Manager with 2-3 years of experience in behavioral health settings. The ideal candidate will be skilled in completing clinical documentation, conducting thorough assessments, and coordinating care to ensure a seamless client intake process.
Key Responsibilities:
Conduct comprehensive intake assessments with new clients, ensuring all documentation is completed accurately and timely.
Complete clinical documentation including:
ASAM (American Society of Addiction Medicine) assessments Biopsychosocial assessments GAD-7 (Generalized Anxiety Disorder) and PHQ-9 (Patient Health Questionnaire) screenings Collaborate with clinical and administrative staff to ensure clients are appropriately placed into services. Maintain up-to-date client records in accordance with organizational policies and regulatory standards. Support clients with resource referrals, follow-ups, and appointment coordination as needed. Adhere to all HIPAA and confidentiality regulations. Participate in case reviews, supervision, and staff meetings. 2-3 years of experience in case management or intake within a behavioral health or mental health setting. Demonstrated proficiency in completing ASAMs, Biopsychosocial assessments, GAD-7, and PHQ-9 . Strong organizational, time management, and interpersonal communication skills. Ability to work independently and as part of a multidisciplinary team. Familiarity with electronic health record (EHR) systems is preferred. High School Diploma or Equivalent CPR/FA Certification

  • to be obtained within first 90 days Successful clearance of pre-employment criminal/county/state background check Successful clearance of pre-employment drug and alcohol testing Successful clearance of TB PPD Test upon hire and annual screening for TB symptoms.
