Job Overview The Case Manager III, Behavioral Health, serves as an experienced team member responsible for advanced case management, crisis intervention, and patient advocacy within behavioral health services. The Case Manager III, is also responsible for conducting patient screenings, organizing and managing referrals, engaging patients in treatment, and ensuring thorough and prompt record keeping. As the frontline responder for patients experiencing behavioral health emergencies, the Case Manager provides direct crisis intervention, de-escalation, and safety planning. In addition, the Case Manager operates within the program's scope of work and is a key resource for engaging complex cases and supporting urgent needs across the department. Minimum Requirements Bachelor's degree in social services, Psychology, Human Services, Counseling, or a related field preferred. A master's degree is strongly preferred. Minimum of 5 years of experience in healthcare, behavioral health, or social services, with a focus on case management, crisis intervention, referral coordination, patient engagement, and care linkage. Bilingual English/Spanish or English and another predominant language of the population served preferred.
