Where Sutter Health Fremont, CA, United States Salary Range (Annual Salary) $94,000 - $127,000
Description Position Overview:
Advanced Certified Hospice and Palliative Social Worker (ACHP-SW) preferred. Responsible for providing specialized psychosocial care and clinical social work services to patients with serious, chronic, or life-limiting illness and their families within a palliative care setting. Delivers evidence-based psychotherapy and supportive counseling, incorporating modalities such as supportive therapy, grief counseling, meaning-centered therapy, and, when appropriate, integrative approaches (e.g., expressive arts or guided imagery). Conducts comprehensive biopsychosocial and spiritual assessments to evaluate the emotional, social, behavioral, cultural, and environmental factors impacting patients and caregivers across the disease trajectory. Addresses complex issues including anticipatory grief, loss, caregiver burden, adjustment to illness, and end-of-life concerns. Provides counseling and education to support coping, resilience, goals-of-care clarification, and informed decision-making. Facilitates advance care planning, including completion of advance directives, POLST, and identification of surrogate decision-makers. Works collaboratively with physicians, nurses, chaplains, and the interdisciplinary care team to support care coordination, symptom management, and treatment planning, serving as the team's primary resource for psychosocial and behavioral health needs. Facilitates family meetings and supports communication between patients, families, and care teams to align care with patient values and goals. Maintains strong knowledge of community resources and provides referrals to hospice, home health, financial, legal, and supportive services as appropriate. Advocates for patients and families across care settings and transitions. Collaborates with social work and behavioral health colleagues to implement and sustain best practices in palliative care and clinical social work, while staying current with evolving research, ethical standards, and evidence-based interventions in hospice and palliative care.
EDUCATION
Master's:
Degree in a Mental Health Discipline
CERTIFICATION & LICENSURE
LCSW - Licensed Clinical Social Worker
TYPICAL EXPERIENCE
2+ years of recent relevant experience
SKILLS AND KNOWLEDGE
Ability to assess the needs of the patient, provide high quality integrative psychotherapy, guided imagery and other evidenced based approaches as indicated Knowledge of holistic assessment, care plan development and outcomes tracking methods Capacity to work within the integrative medicine model as practiced at the Institute for Health & Healing and in line with most recent research findings regarding behavioral health in the ambulatory care setting Candidate must have desire and ability to work as an active member of an interdisciplinary practice team led by physicians Required to have knowledge of the most recent Board of Behavioral Science requirements and adhere to them
