PM Hybrid Schedule:
Training onsite, then 4 days onsite / 1 remote after training completion

- 26/hr Potential opportunity for permanent placement based on openings and performance Qualifications Associate's degree in Social Services, Psychology, Human Services, Counseling, or related field required Bachelor's degree preferred Minimum 2 years of experience in healthcare, behavioral health, or social services required Experience with case management, referral coordination, and linkage to care required Spanish bilingual required Key Responsibilities Coordinate referrals and case management support for behavioral health and substance use patients Assist patients with navigating healthcare systems and accessing community resources Coordinate appointments and follow-up care with behavioral health providers Document patient interactions, referrals, and outcomes within the EHR system Support patients with linkage to care and higher levels of service as needed Answer department calls and provide assistance to patients and providers Collaborate with internal teams, providers, and community agencies to support continuity of care Participate in community events and outreach activities as needed Ideal candidates will have strong communication skills, experience working in fast-paced environments, and a passion for supporting underserved patient populations.
