Position Summary A Step to Freedom is seeking a dedicated and compassionate Interim Case Manager to join our team. The Interim Case Manager plays a critical role in supporting individuals experiencing homelessness and other life challenges by providing comprehensive case management services, crisis intervention, resource coordination, and housing-focused support. The ideal candidate is client-centered, organized, knowledgeable of community resources, and passionate about helping individuals achieve self-sufficiency and long-term stability.

$23.00 - $26.00 per hour (DOE) Essential Duties and ResponsibilitiesCase Management & Client Support Conduct client screenings, assessments, and evaluations to identify needs and barriers. Develop, implement, and monitor individualized service plans designed to support client goals and housing stability. Maintain regular contact with clients and provide ongoing support, guidance, and advocacy. Assist clients in obtaining housing, employment, healthcare, mental health services, substance use treatment, and other supportive services. Facilitate referrals and linkages to community resources and partner agencies. Re-Entry & Life Skills Support Collaborate with clients to establish individualized re-entry, recovery, and life plan goals. Monitor client progress and adjust service plans as needed. Mentor, motivate, and empower clients through a strengths-based and trauma-informed approach. Promote self-sufficiency and independent living skills. Benefits & Resource Navigation Assist clients with applying for and maintaining public benefits, including CalFresh, General Relief, SSI/SSDI, Medi-Cal, and other eligible programs. Educate clients on available community resources and support systems. Advocate on behalf of clients when appropriate. Health & Wellness Coordination Provide education and support related to physical health, chronic disease management, medication adherence, mental health care, substance use recovery, and overall wellness. Coordinate care with healthcare providers and community partners. Documentation & Compliance Maintain accurate, timely, and detailed case notes, assessments, service plans, and client records. Ensure compliance with LAHSA, HMIS, agency, and funding source requirements. Prepare reports, correspondence, and other documentation while maintaining confidentiality and professional standards. Crisis Intervention Respond appropriately to client crises and emergencies. Utilize de-escalation and crisis intervention techniques to ensure client and staff safety. Coordinate emergency services and referrals when necessary. Additional Responsibilities Travel locally as needed to support client services and program requirements. Participate in staff meetings, trainings, and professional development opportunities. Perform other duties as assigned. QualificationsEducation Bachelor's Degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred. Experience Minimum of two (2) years of experience in social services, homeless services, case management, behavioral health, or a related field preferred. Experience working with vulnerable populations, including individuals experiencing homelessness, mental illness, substance use disorders, and justice-involved individuals preferred. Required Knowledge, Skills, and Abilities Experience utilizing LAHSA HMIS
(required). Knowledge of Housing First, trauma-informed care, harm reduction, and client-centered service delivery. Strong crisis intervention and conflict resolution skills. Excellent organizational, communication, and documentation abilities. Ability to maintain professional boundaries and confidentiality. Ability to work independently while collaborating effectively within a multidisciplinary team. Strong knowledge of community resources and public benefits systems. Technical Skills HMIS (Required) Salesforce Microsoft Office Suite Electronic Health Records (EHR) Case Management Systems Preferred Certifications Crisis Prevention and Intervention (CPI) Certification or equivalent training. Motivational Interviewing and Trauma-Informed Care training preferred. Physical Requirements Ability to sit, stand, walk, and move throughout program sites.
