We are seeking a motivated and compassionate Case Manager/Group Facilitator to join our growing team. This role is ideal for someone who is passionate about helping individuals on their recovery journey while providing quality case management and facilitating engaging psychoeducational and process groups. Position Summary The Part-Time Case Manager/Group Facilitator is responsible for coordinating client care, maintaining accurate documentation, facilitating therapeutic groups, and collaborating with the clinical team to ensure clients receive comprehensive support throughout treatment. Minimum Qualifications Current
QMHS , RADT
, or CCDAC (Certified Co-Occurring Disorders Alcohol and Drug Counselor) certification required. Experience working in behavioral health, mental health, or substance use treatment preferred. Proficient in Kipu EMR documentation and electronic health records. Excellent verbal and written communication skills. Strong organizational and time management abilities. Ability to work independently and collaboratively within a multidisciplinary team. Current CPR/First Aid certification (preferred). Valid California driver's license and reliable transportation (preferred). Key Responsibilities Provide case management services to assigned clients. Develop, coordinate, and monitor individualized service plans. Conduct client check-ins and assist with discharge planning and community resource referrals. Facilitate psychoeducational and process groups according to the program schedule. Complete timely and accurate documentation in Kipu , including progress notes, case management notes, group notes, and treatment plan updates. Maintain compliance with agency policies, state regulations, and accreditation standards. Collaborate with therapists, counselors, medical staff, and clinical leadership regarding client progress. Monitor client attendance, participation, and engagement in treatment. Assist with crisis intervention and client support as needed. Participate in staff meetings, trainings, and clinical supervision. Preferred Skills Knowledge of trauma-informed care and motivational interviewing. Understanding of ASAM criteria and substance use treatment practices. Strong interpersonal skills with the ability to build rapport with diverse populations. Ability to maintain confidentiality and professional boundaries. Experience working in residential or outpatient treatment settings is a plus. Schedule Part-Time Flexible schedule, including some evenings or weekends as needed. Compensation Competitive hourly wage based on experience and certifications.
