Job Summary:

We are seeking an Outpatient Case Manager with strong experience in both mental health services and substance use treatment. The ideal candidate understands the unique dynamics of outpatient care and brings a natural ability to motivate, inspire, and meaningfully connect with clients. We are looking for someone who can create a supportive environment that encourages clients to stay engaged in their treatment journey. This specific position offers unlimited growth potential due to Cornerstones rapidly growing Outpatient programming. This role requires a professional who is skilled at building rapport quickly, maintaining client interest, and fostering ongoing participation through approachable communication, clinical insight, and consistent follow through. Your ability to energize clients, reinforce hope, and promote active involvement in the client's recovery is essential to success in this position. Candidates with a passion for client engagement, strong knowledge of Outpatient behavioral health services, and the capacity to support individuals with co-occurring disorders are strongly encouraged to apply.

Responsibilities:

• Conduct comprehensive biopsychosocial assessments to determine the needs, strengths, and barriers for individuals in outpatient care.
• Provide case management for clients with substance use, mental health, and/or co-occurring disorders.
• Develop, update, and monitor individualized treatment plans reflecting client progress and evolving needs.
• Maintain accurate and up-to-date case records, documenting all services provided, changes in clients' conditions, changes to treatment plans, and interactions with clients in company's EMR system (KIPU).
• Educate clients and their families about substance use disorders, treatment options, and community resources.
• Regularly review and update treatment plans to reflect clients' progress and changing needs.
• Assist clients in navigating community resources, aftercare planning, and referrals.

Minimum Qualifications:

CDAC-l or CDAC

• ll (CCAPP Certification), or equivalent.

2 years of outpatient case management experience in mental health, substance abuse, or related field. Bachelor's degree in psychology, sociology, or related field (preferred). Excellent interpersonal and communication skills, with the ability to build strong relationships with clients, families, and other service providers. Ability to handle high-stress situations and make critical decisions under pressure. Strong organizational skills, with the ability to manage multiple cases and prioritize tasks effectively. Proficiency in electronic health records systems and Microsoft Office Suite. EMDR training and/or certification is a plus. Valid California Driver's License required. Individuals in personal recovery must have a minimum of 2 years clean and sober. Must pass a criminal background check as required by licensing authorities.

CPR, AED

& First Aid Certification required. Annual TB Testing required.

Regulatory & Compliance Expectations:

Maintain compliance with all state and federal regulations, including DHCS and Joint Commission standards. Follow company policies on safety, ethical conduct, and confidentiality. Participate in audits, training, and continuous quality improvement efforts.

Compensation Range:

$30

• $35 per hour (commensurate with experience and qualifications).
