In this vital role, you will coordinate comprehensive care plans for individuals with complex health needs, ensuring seamless access to medical, behavioral health, and social services. Your expertise will help improve patient outcomes, promote recovery, and support long-term wellness. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse populations within a collaborative healthcare environment. Duties Conduct thorough patient assessments to identify medical, behavioral health, and social needs using medical terminology and physiology knowledge. Develop individualized care plans that incorporate discharge planning, outpatient services, post-acute care, and inpatient support as needed. Coordinate with hospital teams especially those with Level I Trauma Center, Level II trauma center, ICU, PICU, and emergency medicine experience to facilitate smooth transitions of care. Manage utilization review and utilization management processes to ensure appropriate resource use while adhering to HIPAA regulations. Facilitate intake procedures and maintain accurate medical records for all patients served. Collaborate with multidisciplinary teams including social workers and counselors to provide comprehensive case management and behavioral health support. Support patients through diagnostic evaluation, patient service initiatives, and outpatient clinic experiences to promote recovery and wellness. Skills Proven ability in discharge planning, inpatient/outpatient coordination, and post-acute care management. Strong knowledge of medical terminology, physiology, mental health counseling, and behavioral health principles. Experience working within managed care frameworks and utilizing diagnostic evaluation tools for case management purposes. Familiarity with HIPAA compliance, medical records management, patient assessment techniques, and utilization management protocols. Excellent communication skills for engaging with patients across pediatrics and adult populations in diverse healthcare environments. Ability to navigate complex healthcare systems including hospital medicine settings such as OR experience or PICU expertise is highly desirable. Preferred:
At least 1 year of experience in home health case management, home health field nursing, assisted living, and/or post-acute/SNF settings. Candidates without experience are encouraged to apply; training will be provided. Familiarity with Medicare compliance, guidelines, and approved documentation.
Pay:
$35.00 - $45.00 per hour
