The ideal candidate will play a crucial role in supporting individuals facing various challenges, including documents, housing, employment, behavioral health issues, addiction, and crisis situations. This position involves working closely with clients to develop personalized care plans, facilitate access to resources, and provide ongoing support throughout their journey towards recovery and stability. Responsibilities Conduct comprehensive assessments to identify client needs and strengths. Develop and implement individualized case management plans that address clients' goals. Provide crisis intervention and management services to clients in urgent situations. Offer addiction counseling and support for individuals struggling with substance use disorders. Collaborate with shelter staff and peer coaches to support clients Collaborate with healthcare providers, social workers, and community organizations to coordinate services. Utilize motivational interviewing techniques to empower clients in making positive changes. Maintain accurate documentation of client interactions, progress, and referrals in compliance with regulatory standards. Stay informed about Section 8 housing resources, child welfare programs, Medicare services, and workforce development initiatives to assist clients effectively. Requirements Bachelor's degree in Social Work, Psychology, or a related field; relevant experience may substitute for education. Proven experience in crisis intervention and crisis management within a behavioral health setting. Familiarity with addiction counseling principles and practices is preferred. Strong understanding of social work principles, child welfare systems, and community resources. Excellent communication skills with the ability to build rapport with diverse populations. Experience in supervising or mentoring staff is a plus. Knowledge of workforce development strategies is beneficial. Ability to maintain confidentiality and adhere to ethical standards in all interactions.
