The Social Worker is responsible for providing psychosocial support, facilitating care planning, coordinating services, and assisting residents and families through transitions and discharge planning. This role includes conducting assessments, addressing behavioral or emotional needs, supporting crisis intervention, and collaborating with nursing, medical providers, and the interdisciplinary team to promote resident stability and well-being. Ideal candidates are strong communicators with experience in LTC, mental health, or related fields. Must be comfortable working with residents who have dementia, mood disorders, and behavioral health diagnoses. Experience in social services or healthcare is preferred.

From $20.00 per hour
