Director of Social Work - Long Term Care setting

Job Summary We are seeking a dedicated and compassionate Social Worker specializing in Long Term Care to join our healthcare team. The ideal candidate will possess a strong background in clinical social work, with expertise in providing comprehensive patient-centered support within long-term care settings. This role involves assessing patient needs, coordinating care plans, and delivering psychosocial interventions to enhance quality of life for residents. The successful applicant will demonstrate excellent communication skills, a thorough understanding of healthcare regulations, and a commitment to holistic patient care. Duties Conduct thorough assessments of residents' psychosocial needs through clinical evaluations and interviews. Develop individualized care plans that incorporate behavioral health strategies, mental health counseling, and social support services. Provide individual and group psychotherapy sessions utilizing techniques such as cognitive behavioral therapy, motivational interviewing, and behavioral therapy. Collaborate with interdisciplinary teams including medical staff, nursing personnel, and family members to coordinate comprehensive care. Manage crisis intervention situations effectively, ensuring timely response to urgent mental health or behavioral issues. Maintain accurate medical documentation and records in compliance with HIPAA regulations and healthcare standards. Facilitate discharge planning and connect residents with community resources, hospice care, or specialized services such as Alzheimer's or dementia care. Engage in clinical research activities related to long-term care practices and behavioral health interventions to improve patient outcomes. Skills Extensive experience in social work within long-term care environments such as assisted living facilities or nursing homes. Proficiency in medical terminology, ICD coding (ICD-9, ICD-10), CPT coding, and EMR systems for efficient data collection and documentation. Strong knowledge of mental health counseling techniques including crisis management, PTSD care, working with individuals with developmental disabilities or autism spectrum disorder. Ability to perform diagnostic evaluations and case management tailored to geriatric populations or patients with chronic conditions like dementia or PTSD. Familiarity with hospice care protocols, post-acute care processes, and working with individuals with disabilities across outpatient and inpatient settings. Excellent communication skills for intake assessments, family consultations, and interdisciplinary collaboration. Knowledge of healthcare regulations such as HIPAA to ensure confidentiality and proper handling of medical records.
