JOB DESCRIPTION
SUMMARY
OF POSITION
Provide mental health, substance abuse and outreach services to individuals who may have a mental health and/or substance use disorder. This role will work with community partners and local stakeholders to provide comprehensive care. As a part of the community-based or office-based team, the Case Manager will meet with individuals in the office and/or their own environment such as in the client's home, hospital, schools/work, etc. This role will ensure continuity of care with clients through case management services as well as link clients and their families to needed resources to help to improve their quality of life. Team lead will assist with recruiting new staff, training new staff, error reports, and provides leadership to staff within the department.
ESSENTIAL DUTIES & RESPONSIBILITIES
Directs, administers and assists in control of the day-to-day operations and activities of assigned department working closely with department supervisor to ensure the smooth operation of the department. Provides leadership, support and guidance to personnel of assigned department. Participate in the implementation and development of area strategic plans, goals and objectives to streamline agency processes. Participate in staff meetings and recruiting applicants. Ensures a strong and robust communications process between all management and program staff both within the department and those working in conjunction with the department. Provides individual and group mental health and substance use disorder case management services. Refers, advocates, and links client and family for additional services as identified. Provides supportive and educational counseling services (individual, family, and group) to develop daily living and coping skills. Assist clients with developing natural support systems. Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family, as well as conducting clinical behavioral health assessments. Enters written documentation into electronic medical record in a timely manner for all services rendered. Act as liaison and communicate collaboratively with local law enforcement, and local stakeholders to maintain coordination of the treatment process. Completes all required agency trainings to maintain 100% compliance and ensure all employees receive the appropriate training and education. Work with supervisor to conduct error reports to find and correct errors and ensure proper training can be administered to staff. Maintains departmental productivity expectations. Additional training and/or certification may be required based on specific program guidelines.
KNOWLEDGE, SKILLS & ABILITIES
Good organizational, time management, written and verbal communication skills. Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries. Strong interpersonal communication skills to collaborate with team members and maintain professionalism. Excellent communication, customer service, de-escalation and computer skills and knowledge of mental health and addictions services. Demonstrates clinical skills in working with mentally ill, addicted and emotionally disabled populations. Experience in emergency screening and in providing case management services to the consumers and their families in an outpatient or outreach setting. Excellent telephone and customer service skills required. Proficient with word processing and programs.
QUALIFICATIONS
Education Level Bachelor's Degree in Behavioral Health or related human services discipline required. Years of Experience Experience in a mental health or addictions setting preferred. Knowledge of behavioral health payer system preferred. Bi-Lingual is a plus. Certification/Licensure Valid Florida Driver's License with a safe driving record and current auto insurance.
