Case Manager Position Overview:
The Case Manager is responsible for overseeing and facilitating comprehensive care for clients with mental health needs. This role involves coordinating services, advocating for clients, and ensuring the delivery of high-quality mental health support.
Key Responsibilities:
Daily Tasks:
Client Intake and Assessment:
Conduct initial assessments to determine client needs. Gather relevant background information and develop client profiles.
Care Coordination:
Develop and implement individualized care plans in collaboration with clients. Schedule and facilitate appointments for clients with mental health professionals, including therapists and psychiatrists.
Follow-Up and Monitoring:
Check in with clients to monitor progress and address any emerging issues. Adjust care plans as needed based on client feedback and progress. Monthly Contact & Collaboration required
Resource Management:
Identify and connect clients with community resources and support services (e.g., housing, vocational training). Maintain a resource database for easy access to local services.
Documentation:
Maintain accurate and up-to-date client records, including service plans, progress notes, and case summaries. Ensure compliance with legal and ethical standards regarding confidentiality and client care.
Team Collaboration:
Participate in regular team meetings to discuss client cases and share insights. Collaborate with social workers, healthcare providers, and community organizations to ensure holistic care.
Advocacy and Support:
Advocate for clients' rights.
Crisis Intervention:
Respond to client crises, providing immediate support and intervention and advocacy. Develop safety plans and coordinate emergency services when necessary, based on client's needs.
Training and Development:
Stay updated on best practices in mental health care and case management. Attend training sessions and workshops to enhance skills and knowledge. Attend training to meet the CEU count for the annual Re-Certification.
Qualifications:
Bachelor's degree in Social Work, Psychology, Nursing, or a related field (Master's preferred). Previous experience in mental health case management or a related role.
Skills:
Bilingual:
English & Creole Strong interpersonal and communication skills. Knowledge of mental health issues, treatment modalities, and community resources. Ability to work collaboratively in a multidisciplinary team environment. Proficient in using case management software and electronic health records. Certified BHCM from
FCB Beneficial Pay:
$21.00 - $23.00 per hour
