Job Description

A-Line Staffing is now hiring Hybrid Behavioral Case Manager(Spanish) in located in Candidates must reside in North Miami, Aventura, Hialeah or Miramar Miami Dade County,FL .This would be full time / 40+ hours per week. Hybrid Behavioral Case Manager Hours:
Monday - Friday, 8am-5pm, no weekends, no holidays
Pay:
$34.13 Travel in
County Required:
Travel time is paid at the hourly wage Mileage is reimbursed at the IRS rate
Hybrid Behavioral Case Manager Job Duties:
Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees. Conducts comprehensive evaluation of Members using care management tools and information/data review Coordinates and implements assigned care plan activities and monitors care plan progress Conducts multidisciplinary review to achieve optimal outcomes
Hybrid Behavioral Case Manager Requirements:
Case Management experience required Bachelor's Degree Required (Social Work or related field) Willing and able to travel using your own vehicle 50-75% of their time to meet with members face to face in and surrounding areas (Must have reliable transportation and a driver's license)
Hybrid Behavioral Case Manager Preferred Qualifications:
Long Term Care experience preferred Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment Effective communication skills, both verbal and written Apply today with Alexis Thank you,
