Responsibilities:

The Case Manager is responsible for advocating on the client's behalf with employers, social services agencies, legal systems, or/other involved parties. More specifically assisting patients with FMLA and Short Term Disability to remain gainfully employed once they complete the program. The case manager will help plan the clients transition back to their daily schedules. He or she may assist the clients in finding housing and coordinating discharge plans. He or she also will work closely with the Clinical and Medical team to support client progress in achieving their needs. We offer a competitive salary and benefits. Skills/Abilities:

Ability to work as a team member and have management, communication, organizational and interpersonal skills. Ability to work under stressful conditions and be flexible in relation to department needs. Demonstrates Proficiency in Verbal & Written Communication Skills. Demonstrates Proficiency in Microsoft Office Patient Confidentiality Laws. Knowledge of Drug-Free Workplace Policies. Knowledge of Workplace Violence. Knowledge of Corporate Integrity & Compliance Program.
