• MUST currently reside in one of the following ZIP codes: 33010, 33012, 33013, 33014, 33015, 33016, or 33018 Adecco Healthcare & LifeSciences is looking for a self-motivated, detail-oriented,anized, and tech-savvy Case Management Coordinator to support members throughout the Hialeah area of Miami-Dade County, FL.

In this role, you will conduct telephonic and face-to-face case management visits to assess, plan, coordinate, and support member healthcare needs while promoting overall wellness and quality oues. ✅

Key Responsibilities:

• Conduct member assessments and coordinate individualized care plans
• Manage supportive and medicallyplex caseloads
• Coordinate care with PCPs, specialists, and healthcare providers
• Assist with prior authorizations and medication reviews
• Connect members withmunity resources and support services
• Monitor short
• and long-term health oues
• Maintain accurate documentation and case management records
• Provide condition management education and wellness support ✅

Ideal Candidate:

• Strong case management and care coordination experience
• Excellentmunication andanizational skills
• Comfortable working independently in the field
• Tech-savvy with strong documentation skills
• Bilingual English/Spanish required

Pay Details:
