  • No •The Case Manager is responsible for:•+ Case Managers provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, and/or those whose involvement in the child welfare system requires.
+ The Case Manager should demonstrate competency in the domains of Engagement and Assessment; Service Planning and Delivery; Coordination, Linkage and Monitoring; Documentation; and Professional Responsibility. + The Case Manager coordinates the service delivery for individuals, and provides community partners with up to date available referral resources to provide continuity of care. + Case Managers effectively maintain and manage their assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served. + The Case Manager possess a clear understanding of the service delivery system in accordance with regulations and district policy. + Case managers provide services at the time and location that is most appropriate to the person served. Travel is required. + Case Managers may also be required to conduct urine drug screening and transport clients, depending on the program. + Some programs may also require a 24 hour on-call rotation. Case Managers will perform other duties as assigned.
  • Required Education:
  • + Bachelor's degree related to related fields, Social Work, Counseling, Psychology Criminal Justice, Health Education, Special Education OR Other Human Services Related Field.
  • Required Experience:
  • + For targeted case managers working with children have 1 year full-time experience working with children with behavioral health concerns. + For targeted case managers working with adults have 1 year full time experience working with adults with behavioral health concerns. +
  • Preferred Experience
  • : 1 year of full-time experience with the identified population
  • Required Certification/License:
  • + BLS (Basic Life Support) + Driver's License + For targeted case managers (CBHCM) Certified Behavioral Health Case Manager certification must be obtained through the Florida Certification Board (FCB) within one year of employment. Preferred for all case managers. +
  • Targeted Case Managers required to bill Medicaid.
