There is a place for you at Highland Rivers Behavioral Health! Highland Rivers provides treatment and recovery services to individuals impacted by mental illness; substance use disorders and intellectual developmental disabilities.
Job Summary:
Highland Rivers Behavioral Health is committed to providing services for the Veterans with mental health and addictive diseases in our area. As part of this, the Case Manager will assist with linkage to appropriate services for ongoing treatment, accessing resources within the community, skill building around crisis reduction and learning skills to reduce the impact of their mental health symptoms and/or substance use on their recovery. Job Duties and Responsibilities:
Provide case management linkage for resources and education on skills to reduce the impact of their mental health symptoms or substance use on their recovery. Meets with participants to update treatment plans, risk assessments, safety plans and release of information forms; Attend staffing and treatment team meetings with other service providers to assist the team in providing the needed services for the individual. Assisting participants with scheduling appointments and accessing resources within the community. Meet with participants and constituents in the community to represent and promote the veterans program. Is available to handle crisis telephone calls, information seeking (about our services) telephone calls and follow up engagement/outreach calls. Provides transportation for individuals as needed. Other duties as assigned.
