This full-time position focuses on helping individuals build stability, strengthen independence, and access the behavioral health, housing, medical, and community resources needed to support their recovery goals. As a Case Manager, you will coordinate care, advocate for client needs, and provide recovery-oriented support that empowers individuals to live healthier, more independent lives. This role is ideal for compassionate professionals in Honolulu and throughout Oahu who enjoy relationship-based work and making a meaningful impact within their community. What You'll Do (Responsibilities)
Care Coordination:
Assess client needs, develop individualized service plans, and coordinate behavioral health, medical, housing, and community resources.
Client Advocacy:
Support individuals in accessing benefits, services, and community-based supports that promote stability, wellness, and independence.
Recovery-Oriented Support:
Utilize person-centered, strength-based, and trauma-informed practices to help individuals achieve their recovery goals.
Hospital & Transition Support:
Provide follow-up services after psychiatric hospitalization and support successful transitions between levels of care.
Community Collaboration:
Partner with healthcare providers, families, landlords, and community organizations to coordinate comprehensive support.
Client Transportation:
Transport clients to appointments and essential services using your personal vehicle as needed.
Documentation & Compliance:
Complete timely assessments, service plans, progress notes, and documentation that meets agency, contract, and billing requirements.
Team Collaboration:
Participate in supervision, case reviews, and interdisciplinary team meetings to ensure high-quality, recovery-focused services.
Case Manager Qualifications Preferred:
Bachelor's degree in psychology, sociology, social work, human services, or a related behavioral health field.
Preferred:
One and a half (1.5) years or more of case management, human services, behavioral health, or mental health experience; experience working with individuals experiencing co-occurring disorders is a plus.
Skills:
Strong communication, organization, documentation, advocacy, and relationship-building skills.
Certifications:
Current TB clearance, CPR, and First Aid certification (or willingness to obtain). Fit for the

Requirements & Work Conditions Ability to pass a criminal background check and drug/alcohol screening. Valid Hawaiʻi driver's license, current vehicle insurance, and safety inspection. Ability to use your personal vehicle to transport clients as needed. Ability to travel throughout Oahu for client visits, appointments, meetings, and community-based services. Ability to lift up to 25 pounds and respond to emergencies as needed.
