Job Description

UPON CONTRACT AWARD
•Job Summary The Behavioral Health Case Manager provides comprehensive case management and care coordination services to support the Military personnel. This role is critical to ensuring Soldiers are mentally prepared and connected to community and military resources, supporting their successful adjustment between civilian and military life. The position requires flexibility, responsiveness, and a commitment to maintaining strength and readiness. Duties Coordinate mental fitness and wellness for operational readiness, developing systems to identify and resolve behavioral health concerns. Conduct bio-psycho-social assessments, including substance abuse and risk assessments, using standardized tools or equivalent (e.g., PLC-M for
PTSD, PHQ-9
for depression, Columbia for suicide risk, GAD-7 for anxiety, AUDIT for alcohol abuse). Provide mental health assessments, screenings, and referrals throughout all phases of the deployment cycle. Develop and implement marketing and educational strategies for behavioral health programs. Collaborate with the State Behavioral Health Programs Team and other programs to coordinate care and resources for Service Members and their families. Maintain accurate and timely documentation in electronic health records (EHR) in accordance with Army regulations. Prepare and submit formal case summaries, narrative summaries (NARSUM), and monthly reports to the Director of Psychological Health (DPH). Attend required events such as Periodic Health Assessments (PHA), Soldier Readiness Processing (SRP), and outreach activities. Identify and monitor high-risk Service Members, ensuring regular contact and reporting. Ensure compliance with HIPAA, Army regulations, and all applicable laws and policies. Other duties as required
Qualifications:
Master's degree in Social Work (MSW) from a CSWE-accredited institution or preferred equivalent Licensed Master Social Worker (LMSW) level or equivalent is preferred. On a case-by-case with approval, a bachelor's degree in a relevant field and a minimum of 3 years of relevant work experience may be substituted for a master's degree. A bachelor's degree in a relevant field with 3+ years of experience may be considered with approval. Current state license or certification as a mental health practitioner (e.g., social worker, clinical psychologist, marriage and family counselor, certified drug and alcohol counselor, or professional counselor) in Iowa.
Continuing education:
2 hours of substance abuse and 1 hour of suicide-related training biennially. Ability to practice independently without state-mandated clinical supervision. Minimum of 3 years of clinical or administrative mental health care experience post-graduate degree. Proficient in Microsoft Office Suite and electronic client management systems. Experience with Army/Air Force medical regulations and working with military personnel and community agencies.
Desired Skills:
Strong organizational, written, and verbal communication skills. Experience in program development, marketing, and public education. Ability to interpret general direction and use various software programs for database management and reporting. •

$27.00 - $38.00 per hour
