General Functions:
Provide case management and community mental health services to individuals served. Population of focus is adults with a serious mental illness and/or co-occurring substance use disorder living in the community.
Specific Functions:
Deliver facility-based and community-based behavioral health and recovery support services in accordance with HFS Rule 140, DMH Rule 132, DBHR Rule 2060, and individual treatment and recovery plans. Services include case management, linkage and referral, outreach, advocacy, and community support. Assist individuals in accessing prenatal and postpartum care, pediatric services, mental health treatment, substance use treatment, housing, transportation, public benefits, parenting resources, and other essential community supports. Provide psychoeducation and recovery-oriented support related to parenting, emotional regulation, relapse prevention, trauma recovery, attachment, independent living, and family stabilization. Follow all agency and department safety guidelines when providing services in the community and in the individuals homes. Record the process and results of services within established timeframes and in sufficient detail to meet HFS Rule 140/DMH Rule 132/DBHR Rule 2060 and/or other insurance billing requirements. Meet and/or exceed assigned MRO weekly billing quota as determined by the Director. Interview, hire, train, evaluate, and discipline staff in accordance with personnel policies and procedures, under the direction of the Manager. Provide supervision and guidance to the assigned staff, ensuring the delivery of the indicated services in accordance with established policies and procedures. Responsible for the management and maintenance of the persons served electronic case records when serving as primary case manager. Responsible for the provision of services outlined in the Individual Treatment and Recovery Plan. These may include case management-mental health, community support-individual/group, case management-client centered consultation, crisis intervention, and medication training-individual. Participate in interdisciplinary staffing's and coordinate regularly with treatment team members, families, guardians, medical providers, DCFS, and community partners to ensure continuity of care and integrated service planning. Communicate on a regular basis with other program staff to ensure consistency in training, treatment, and facilitation of the integrated Individual Treatment and Recovery Plan. Participate in monthly clinical supervision meetings, seek ongoing clinical supervision from supervisors, and participate in continuing education to maintain applicable licenses/certifications. Participate in all agency and department trainings, monthly department meetings, and any additionally recommended trainings or meetings as required by supervisor(s). Maintain ongoing compliance with all agency-related training and certification requirements. Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards. Operate agency or personal vehicle in accordance with established policies and procedures. Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures. Other duties as assigned.
Qualifications:
Consistent with requirements for Mental Health Professional, bachelor's degree in related field, practical nursing license or minimum of five years supervised experience in mental health/human services. Bachelor's degree in related area with experience in mental health and substance use is preferred. Experience or interest in maternal-child health, perinatal support, trauma-informed care, and/or community-based services strongly preferred. Knowledge of trauma-informed care, recovery-oriented systems of care, and community resource navigation preferred. Proficiency with Microsoft Office (Word, Excel, Outlook) and electronic health record systems required. Strong communication, documentation, and organizational skills required. Driving record and auto insurance coverage in compliance with agency policy. A criminal background check and post-offer drug screen required. Reliable transportation required. EOE.
