Job Description

Schedule:
Wednesday - Saturday | 10:00 AM - 6:30 PM 32 Hours per
Week Position Summary:
We are seeking a compassionate and dedicated Case Manager to provide case management services for individuals undergoing detoxification and substance use treatment. This role is responsible for assessing client needs, coordinating care plans, supporting recovery efforts, and connecting individuals with continuing care resources. The ideal candidate will possess strong communication skills, crisis management abilities, and experience working with substance use populations.
Key Responsibilities:
Manage an assigned caseload, including conducting intake evaluations and ongoing assessments Develop treatment plans, aftercare plans, safety plans, and coordinate discharge planning Provide medication management support and assist with medication retrieval from local pharmacies Complete documentation accurately and within required timeframes Coordinate with referral sources, community partners, and collateral contacts as needed Assist with transportation coordination for appointments and aftercare services Support staff with milieu management, crisis intervention, and de-escalation techniques Maintain access to required systems and complete required assessments and documentation Conduct unit searches and assist in identifying prohibited items when necessary Assess continuing care needs and coordinate placement into appropriate aftercare programs Participate in educational groups, recreational activities, and wellness programming Attend staff meetings, supervision sessions, and trainings as required Provide support resources and information to affected family members when appropriate Perform additional related duties as assigned
Qualifications:
Education & Experience:
Bachelor's Degree required OR Associate's Degree in Human Services or related field 2-3 years of experience working in chemical dependency, substance use treatment, behavioral health, or related field preferred
Preferred Qualifications:
Certified Addiction Counselor (CAC) certification preferred Experience working within detox, residential, or behavioral health settings preferred Knowledge of community resources for continuing care and aftercare placement

Strong interpersonal, counseling, and communication skills Ability to work effectively with multicultural populations Experience managing difficult or emotional situations professionally Excellent conflict resolution and crisis management skills Strong documentation and organizational abilities Ability to maintain confidentiality and professional boundaries Customer service focused with a compassionate and patient-centered approach Strong problem-solving and critical thinking skills
Additional Requirements:
Must be able to complete on-site CPI and CPR training/certification Must demonstrate sensitivity and professionalism when working with substance use treatment populations Ability to respond effectively to client concerns, regulatory inquiries, and service coordination needs #MED1 #
Behavioral Health Case Manager Pay:
$29.00 - $31.00 per hour
