Job Description

RESPONSIBILITIES
Work with a multidisciplinary team including physicians, advanced practice clinicians, nurses, and case managers Assess, diagnose, and develop treatment plans for all clients, including knowledge of medication-assisted treatment options Develop measurable, client-centered treatment goals Provide trauma-informed, person-centered care using evidence-based practices such as motivational interviewing, CBT, and harm reduction Identify and connect clients to appropriate community resources (e.g., treatment programs, AA/NA, mental health supports) Educate clients on harm reduction, overdose prevention (including naloxone), infection risk reduction, and HIV prevention strategies Support women and families through the childbearing process by assessing strengths and challenges Assist with accessing long-term supportive services Promote healthy parent-child relationships Advocate for clients within healthcare and community settings Facilitate individual and group therapy for peripartum women Maintain ethical and professional standards in accordance with applicable guidelines Ensure confidentiality in accordance with legal and professional standards Address systemic factors impacting client functioning Manage client safety concerns and emergencies appropriately Participate in supervision, training, and staff meetings Maintain accurate and timely documentation Follow all organizational policies and procedures Participate in quality improvement initiatives • We will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state, and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO), and the California Fair Chance Act (CFCA). • As a job position within our Insurance division, a successful completion of a background check may be required as a condition of employment.
