Job Description

We are seeking a compassionate, organized, and experienced Nurse Care Manager to join our Substance Use Disorder (SUD) Health Home team serving individuals throughout the Metro Detroit area. This position is based out of our Eastpointe, Michigan office and focuses on coordinating comprehensive healthcare services for individuals with substance use disorders, co-occurring mental health conditions, chronic medical needs, and social determinants impacting recovery. The Nurse Care Manager will work collaboratively with counselors, physicians, case managers, peer recovery coaches, hospitals, community mental health agencies, primary care providers, and social service organizations to improve patient outcomes, reduce hospitalizations, support recovery, and ensure continuity of care. Candidates may be either a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with experience in behavioral health, addiction medicine, case management, community health, or care coordination. Essential Duties and Responsibilities Provide nursing care coordination and case management services for individuals enrolled in the Health Home Program. Conduct comprehensive nursing assessments, medication reviews, health screenings, and care planning. Coordinate medical, behavioral health, and substance use treatment services. Monitor and track patient progress, treatment compliance, appointments, and follow-up care. Educate patients regarding medications, chronic disease management, substance use disorders, overdose prevention, infectious diseases, and wellness practices. Assist clients in accessing community resources including: Primary care services Psychiatric services

services Housing resources Medicaid and insurance navigation Transportation services Food assistance programs Specialty medical care Collaborate with multidisciplinary treatment teams to develop and update individualized care plans. Coordinate discharge planning and transitions of care following hospitalization, detoxification, residential treatment, or incarceration. Maintain accurate and timely clinical documentation in accordance with Michigan Medicaid, Health Home, CARF, and agency requirements. Participate in case conferences, quality improvement activities, and utilization reviews. Conduct outreach and engagement activities for high-risk or difficult-to-engage clients. Monitor health indicators and support preventive healthcare measures. Maintain confidentiality and compliance with HIPAA and 42 CFR Part 2 regulations. Support crisis intervention and referral processes when necessary. Qualifications Required Qualifications Active Michigan nursing license as one of the following: Registered Nurse (RN), or Licensed Practical Nurse (LPN) Minimum of one (1) year experience in one or more of the following: Substance Use Disorder treatment Behavioral health Community mental health Care management Case management Primary care Public health Knowledge of substance use disorders, co-occurring disorders, and recovery-oriented systems of care. Strong communication, organizational, and documentation skills. Ability to work effectively with underserved and vulnerable populations. Proficiency with electronic health records (EHR) systems and Microsoft Office applications. Valid driver's license and reliable transportation for community-based services and outreach.
