This role is part of a dedicated interdisciplinary team that provides primary care services through street medicine and place-based clinics embedded in partner organizations. Utilizing a trauma-informed and client-centered approach, the HCH Case Manager conducts assessments, facilitates service navigation, coordinates healthcare needs, and connects clients to appropriate community resources. : Catherines Health Center Clinic at

  • Preferred clinical experience;
MA/LPN/RN
  • Patient Assessments
  • conducts thorough assessments of clients' health, social, and housing needs to develop individualized care plans or referrals to resources
  • Care Navigation
  • Assists patients in navigating healthcare systems, including scheduling appointments, medication management, transportation arrangements, and follow-up care.
  • Transitions of Care
  • leads efforts to support transitions of care from inpatient hospitalization and emergency department care for individuals experiencing homelessness who require an integrated primary care medical home.
  • Care Coordination
  • Supports health-focused case conferencing efforts that ensures that people experiencing homelessness avoid fragmented services. The HHCM will work with partner hospitals, healthcare providers, community mental health, and social service providers to coordinate care that avoids duplication of services.
  • Monitoring and Evaluation
  • Regularly assesses patient progress and updates care plans as needed.
  • Other duties as assigned.
  • Preferred term is unhoused.
Catherine's receives "Healthcare for the Homeless" funding from HRSA Minimum Position Qualifications Individuals with lived experience are encouraged to apply Experience or formal education in social work, nursing, public health, psychology, healthcare administration, or sociology. 3 years of experience working in community health, public health, social work, healthcare delivery to disenfranchised populations, or directly with a homeless population is required. Experience with serving individuals experiencing homelessness or other population with complex health and social services need is required A passion for connecting with partners who serve people experiencing homelessness is required Experience with client-centered approaches to care such as Housing First and Harm Reduction (preferred) § Experience with crisis intervention (preferred) Please note that this position requires a comprehensive background check as part of our commitment to patient safety and organizational integrity. Commitment to health equity and addressing health disparities Superior verbal, written, analytical, and interpersonal skills to facilitate interactions with all levels of management and program staff. Ability to manage and various relationships and relate well to people from diverse ethnic and cultural backgrounds, as well as have a passion for working with culturally diverse populations Basic understanding of information technology and ability to organize, analyze and synthesize data from various sources Must have excellent process improvement skills and able to understand clinic functions and department interactions Excellent computer skills utilizing Microsoft Suite of products, and computerized accounting and database software, including but not restricted to MS Access and advanced features of MS Excel, with a passion for data manipulation Able to adapt process improvement in accordance with organization objectives; and Willingness to work flexible hours in order to meet the organization's needs/demands. Must maintain strict confidentiality.
