QUALIFICATIONS
LMSW, LLMSW, LBSW, LLBSW, or LLPC required for this position OR qualify as a QMHP (Qualified Mental Health Professional) related human services degree (social work, community health, human services, psychology, criminal justice, etc.) and at least 2 years experience in mental health or case management work. Strong written and verbal communication skills, valid driver's license and reliable transportation, flexibility in treatment approach, willingness to work with complex cases involving physical health, mental health, IDD, and legal issues.Job Description SUMMARY:
This is a full-time on-site role as a Case Manager at Case Management of Michigan, Inc. The Case Manager will be responsible for providing intensive, focused support to clients with mental illness, facilitating community integration, engaging with various stakeholders, and maintaining small caseloads for personalized care.DUTIES AND RESPONSIBILITIES
Facilitates client access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services; assists client to develop natural resources and make contact with social support networks. Coordinates and monitors psychiatric services and supports, including comprehensive tracking of client activities in relation to care plan. Assists clients in developing goals and areas of need, and assists in developing treatment plans, which are assessed regularly; conducts medication and mental status assessment and determines required level and frequency of services. Documents all client encounters and contacts made on behalf of clients; completes and submits billing documentation as appropriate; maintains comprehensive client files, which may include documents held for safekeeping on behalf of the client. Identifies and provides emergency crisis services as necessary; makes immediate clinical assessments and responds according to accepted crisis intervention methods and techniques; coordinates other services as appropriate. Assesses clients, evaluates effectiveness of care plan and progress made by client, participates in treatment planning and case review with patient care providers. Maintains and reports applicable statistics regarding programs and client services. Performs miscellaneous job-related duties as assigned.Minimum Job Requirements:
Bachelor's degree required LLMSW, LLPC, LMSW, LBSW or LLBSW required for this position OR qualify as a QMHP (Qualified Mental Health Professional) related human services degree (social work, community health, human services, criminal justice, etc.) and QIDP (Qualified Intellectual Disability Professional) At least 1 year experience in mental health or case management work.Conditions of Employment:
Specialty licensure/certification may be required, as specified by the department.Working Conditions and Physical Effort:
Work is primarily performed in a typical office environment as well as in Adult Foster Care homes. Some travel is required (up to 50%), reliable transportation is necessary. Minimal physical effort is required. The work environment involves limited exposure to physical risks, such as operating dangerous equipment or working with chemicals.Additional Benefits:
