Job Description

Do you have a passion for improving the health of individuals receiving behavioral health services? Would you like to be a valuable member of a diversity driven, mission-minded team that impacts our community? If so, we have an exciting opportunity for you to work as a Full-Time (40 hours/wk) Housing Case Manager with the MIBHT team. What You Will Be Doing Working as a full-time Housing Case Manager on our MIBHT team , you will support individuals in locating and maintaining stable housing. You will provide linking, advocacy, coordination, and monitoring services to adults with serious mental illness and/or substance use disorders who are enrolled in an ISK case management program. In this outreach-based role, you will deliver culturally responsive services while collaborating closely with multiple public service systems, including other ISK programs, the Kalamazoo Court System, DHHS, and local hospitals. You will partner with other ISK service providers to coordinate care and ensure clients receive comprehensive support. You will also be responsible for completing clinical documentation accurately and in a timely manner to support quality care and program requirements. What We Require You will have a minimum of one year of experience working with adults with serious mental illness, substance abuse, and/or co-occurring disorders who also have co-morbid health conditions. You will have a Bachelor's degree in Psychology, Social Work, or a related field. A valid Michigan Motor Vehicle Operator's license and reliable transportation are required. Licensed Bachelor Social Worker (LBSW) preferred. Experience supporting individuals with housing search, placement, and/or housing stability is preferred. Working Conditions:
Must be able to perform essential job functions with or without reasonable accommodations, including, but not limited to, visual and/or audiological appliances and devices to increase mobility. Physical Efforts - Job demands include prolonged sitting and standing. Requires light lifting up to 25 pounds, stooping, kneeling, crouching, or bending. Requires coordination of hands and/or eye/hand/foot and ability to hear and communicate verbally . Working Conditions - In office environment with noise from computers, copy machine, and telephones. Use of computer for periods in excess of 30 minutes at a time. May be exposed to bloodborne pathogens, infectious diseases, and parasites. May encounter community environments that include homes, schools, courts, and other community settings that may or may not be barrier-free. Travel throughout the Kalamazoo area may be required.
