Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. We serve all populations regardless of their ability to pay. Our team is passionate about serving the people of Oakland County.
The Level I Targeted Case Manager provides targeted case management services within a Certified Community Behavioral Health Clinic (CCBHC). This role assists individuals in sustaining mental health and substance use recovery and gaining access to needed medical, behavioral health, social, educational, legal, and community services. The targeted case manager focuses on connecting individuals to services that help them resolve problems interfering with their independence or self-sufficiency and maintain themselves in the community. Our standard hours are Mon-Fri, 8:30am-5:00pm. What are we looking for? A bachelor's degree in social work, psychology, nursing, human services, or a related field including state licensures and registration as an RN. At least one year of experience in providing direct services to clients with mental health and/or substance use disorder symptoms. Experience working for a CCBHC or Federally Qualified Health Center preferred. Foundational knowledge of behavioral health systems and community resources. What do we Offer?
