The Care Management Social Worker provides services to customers and their families/significant others, to help them adjust to illness, injury or hospitalization. Assists the health team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes. Responsible for the psychosocial assessments and discharge planning for customers with complex psychosocial and routine medical problems in collaboration with the responsible Case Manager. Facilitates psychosocial coordination including assessment, planning, intervention and evaluation of customer/family needs throughout the hospital stay. Identifies needed community services and coordinates appropriate services for customers in need. In addition to the above position purposes, Social Workers are available to assess the behavioral and psychological needs of customers. They take into consideration a customer's presenting concerns in cases related to psychiatric disorders, chemical health issues and emotional/behavioral health. Clinical Social Workers utilize a brief therapy model and provide counseling/psychotherapy services to individuals and groups. They are responsible for the discharge planning and case management for the customers they are assigned. Clinical Social Workers are encouraged to develop expertise in a variety of areas and to participate in further professional development.Palliative Care:
The Palliative Care Social Worker serves as an integral member of a self-directed inpatient interdisciplinary palliative care team. Provides comprehensive psychosocial assessments, serves as the mental health professional for the palliative care team, and provides appropriate social work interventions to meet the complex needs of customers and families/significant other's in the hospital settings. The primary focus is to assist customers and families/caregivers in coping with in the context of serious and life-limiting illness. Psychiatry & Integrative Care Unit 2E:
The Psychiatric Social Worker serves as an integral member of a self-directed inpatient interdisciplinary psychiatric treatment team. Provides comprehensive biopsychosocial assessments, serves as the mental health professional for the psychiatric inpatient team, provides appropriate social work interventions and case coordination for customers with an acute psychiatric crisis and chemical dependency needs. Facilitates civil commitment processes. Requirements Education- Masters or Doctoral degree from an accredited school of social work required Experience
- Social workers who are required by clinical assignment to psychiatrically diagnosis or engage in the practice of psychotherapy shall have a minimum of 4000 hours of post-graduate clinical social mental health professional as defined by Minnesota statute
- Hospital social work experience preferred.
- Palliative Care only: Prior Palliative Care experience preferred.
- Psychiatric social work only: Experience with Serious Persistent Mental Illness (SPMI) population, mental health diagnoses.
- Knowledge of current case management principles, length of stay management, and/or transition/discharge planning highly preferred.
- Excellent verbal and written communication skills required.
- Knowledge of current social work principles, techniques and values.
