Full Job Description Adult Mental Health Case Management Team Lead Location:

M-F Description:
The Adult Mental Health Case Management Team Lead is a hybrid position consisting of administrative support and direct client service provision. The mental health case management team's primary goal is working with individuals with serious and persistent mental illness (SPMI) and connecting them with the services and resources they need to meet their mental health goals. The Team Lead for Mental Health Case Management will maintain a partial case load of 15 clients and will also provide administrative support in the areas of monitoring compliance with team service provision standards and outcomes. Our program mission is to provide high-quality, person-centered services to individuals in support of their mental health, growth, and independence. In this position, you will: Complete a functional assessment (FA) and individual community support plan (ICSP) on new clients within 30 days of initial meeting Provide ongoing assessment of the client's needs, goals, and the impact of the mental illness Refer and link clients to mental health resources, services, and other supports Monitor the delivery of services and treatment team accountability and involvement Meet with clients at least one time per month, in their homes or in other community settings Ensure appropriate coordination of care for clients through collaboration with other professionals and supports Maintain complete and organized electronic mental health records Attend and participate in regular clinical supervision meetings to discuss the progress made toward goals and recovery for each client Monitor program compliance with documentation requirements, including but not limited to tracking documentation deadlines, ensuring all documentation has met statutory guidelines, and all discrepancies are recorded and communicated to program director Monitoring data entry in EHR system and reporting of necessary updates or issues Monitoring insurance coverage for clients in the program, tracking efforts to assist with obtaining and maintaining coverage Tracking annual program outcomes Weekly meetings with program director for program updates and development Requirements Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field. 2.000 hours of supervised service to adults with mental illness OR 40 hours of training approved by the Behavioral Health Division One year minimum experience as a Adult Mental Health Case Manager One year supervisory experience Demonstrated knowledge of mental health conditions Experience with patient advocacy, care coordination, and treatment planning Ability to adapt and respond in high-pressure situations Excellent interpersonal and active listening skills Proficient in Microsoft Office and other technology applications High-level organizational and time management skills Able to pass a DHS (Department of Health Services) background check, valid driver's license and clean driving record Reliable transportation
