As a Suicide Prevention Liaison, you will play a critical role in improving response times for high-risk clients, engaging in meaningful follow-up care, advancing prevention efforts, and promoting education that saves lives. In this role, you will manage and support the agency's suicide care pathway, conduct follow-ups with clients, collaborate closely with clinical teams and community partners, and ensure accurate tracking and documentation of services. You will also contribute to suicide prevention education and participate in crisis response efforts, helping to ensure that clients receive timely, coordinated, and effective care. Key Responsibilities:
Coordinate and manage the agency's suicide care pathway, including conducting timely and supportive follow-up calls with clients at risk Provide compassionate crisis intervention services via phone and face-to-face to support individuals experiencing suicidality Collaborate with treatment teams, hospitals, law enforcement, and community partners to ensure continuity of care and client safety Connect with service providers regarding safety planning and assist in coordinating appropriate interventions Educate staff and community stakeholders on suicide prevention, including risk factors, warning signs, and intervention strategies Monitor, track, and analyze data related to suicidality outcomes and ensure accurate reporting in the electronic health record (EHR) Participate in Zero Suicide Committee meetings and Ad-Hoc Risk Management/Death Reviews to support continuous quality improvement Work closely with Quality Management to evaluate outcomes and enhance prevention strategies across client populations Maintain thorough, timely documentation and ensure compliance with agency policies and procedures Attend meetings and contribute feedback to improve program effectiveness and service delivery Education, Experience, and/or
Credential Qualifications:
Bachelor's degree in a human services field required (Master's degree preferred) Knowledge and skills in suicide risk assessment, crisis intervention, and triage Strong written and verbal communication skills Demonstrated ability in problem-solving, clinical judgment, and time management Experience using electronic medical records (EHR) systems preferred CPR/First Aid and CPI certification required
Additional Qualifications:
Strong initiative and ability to take appropriate action independently Ability to adapt effectively in a fast-paced, changing environment Valid driver's license, acceptable driving record, and current auto insurance required Successful completion of background checks including criminal, driving, abuse/neglect, and fingerprint screenings required
Physical Requirements:
Perform sedentary work, including exerting up to 10 pounds of force occasionally Frequently use hands and fingers for typing, writing, and data entry Sit for extended periods with occasional standing or walking Perform repetitive movements with hands, wrists, and arms throughout the workday
