Job Description
Overview Helena Valley Health Services is looking for a Mental Health Case Manager and become a vital advocate for patients navigating complex behavioral health needs. In this role, you will coordinate comprehensive care plans, facilitate patient assessments, and connect individuals with appropriate outpatient, inpatient, or post-acute services. Your expertise will help improve patient outcomes through compassionate support, meticulous discharge planning, and effective utilization management. Duties Coordinate inpatient and outpatient services, including hospital medicine, behavioral health programs, and community resources. Manage discharge planning processes to ensure seamless transitions from hospital to home or post-acute care settings. Collaborate with multidisciplinary teams in Level I Trauma Centers, Level II trauma centers, PICUs (Pediatric Intensive Care Units), and emergency medicine units to optimize patient care. Oversee utilization management by reviewing medical records, diagnostic evaluations, and medical terminology to support appropriate resource allocation. Facilitate intake procedures for new patients, including mental health counseling and addiction counseling as needed. Maintain strict compliance with HIPAA regulations and ensure accurate documentation within medical records systems. Support patient service initiatives by providing compassionate case management that addresses physiology knowledge, patient care needs, and social determinants of health. Experience Proven experience working in hospital settings such as ICU (Intensive Care Unit), emergency departments, or acute care units. Prior involvement in behavioral health or mental health counseling within inpatient or outpatient clinics. Familiarity with managed care processes, utilization management protocols, and post-acute care coordination. Experience with social work practices related to patient assessment, discharge planning, and medical records management. Knowledge of medical terminology relevant to hospital medicine, trauma centers, PICUs, and addiction treatment environments. Strong understanding of HIPAA compliance standards and confidentiality protocols in healthcare settings. Demonstrated ability to handle complex cases involving physiology knowledge and multidisciplinary collaboration across various hospital units including pediatrics when applicable.
