Job Description

Job Summary Join our dedicated healthcare team as a Mental Health Targeted Case Manager, where you will play a vital role in coordinating comprehensive mental health services for diverse patient populations. This dynamic position involves working closely with individuals experiencing behavioral health challenges, ensuring they receive personalized, effective care at an outpatient setting providing care in their home and community-based settings. Your expertise will help facilitate seamless transitions between care levels, promote recovery, and support individuals' overall well-being. Duties This position can be a Registered Nurse or a Licensed Social Worker, both positions are open. It is paid on commission, you make 60% of the services you provide. Employees must work over 30 hours a week to be eligable for benefits. If you work less than that it is okay, you just will not get benefits. This position is driven by self-motivation, genuine care for the individuals we serve and having a passion for making a difference. This position is responsible to provide services to assist individuals in gaining access to needed medical, social, educational, and other services necessary for appropriate care and treatment. This position functions as part of an integrated team, providing individuals opportunity for recovery management to reduce the frequency, intensity, and duration for functional impairment. Services may be provided in-person. Most of this position's time will be spent in the community providing outreach services in the client's home and other preferred locations. The majority of the job is in the community, or in the homes of the individuals we serve. The perfect candidate can work independently, manage their time well, and enjoys setting their own schedule. Must have strong compliance with charting daily on the services you provided. Conduct thorough individual assessments to identify mental health needs and develop tailored care plans. Assist in the development and periodic revision of the Treatment Plan of Care. Coordinate services across variety of levels of care and provide community resources to ensure continuous patient support. Collaborate with multidisciplinary teams including social workers, physicians, addiction counselors, and behavioral health specialists to optimize treatment strategies. Complete therapeutic referrals, both internally and externally, to help a client obtain needed services to address identified needs and achieve goals specified in the Treatment Plan of Care. Monitoring and follow-up activities include making necessary adjustments in the Treatment Plan of Care and service arrangements with providers. Maintain accurate medical records in compliance with HIPAA regulations and Cambian Behavioral Health policies. Provide patient education on treatment options, medication management, and coping strategies to empower individuals in their recovery journey. Experience Prior experience with a focus on behavioral health or mental health counseling is highly preferred. Maintain a well-organized work area with a business-like appearance. Use effective communication techniques to develop and maintain positive and effective working relationships with other employees and customers. Be responsive to requests for service and assistance from all customers. Knowledge of the principle concepts needed to perform and/or direct duties related to the care and treatment of the mentally ill. High degree of interpersonal skills to be able to communicate with, motivate and/or facilitate behavioral changes in others. Knowledge of development and dynamics of family and human interactions.. Knowledge of local resources available to clients in the community. Working knowledge of community services. Computer literacy to include Microsoft Office programs. Knowledge of Medicaid Programs.
