Our case manager provides mental health services to children and adolescents as a part of a office/community-based treatment team. The position provides an opportunity for rewarding work with families in need of mental health support, consultation with dedicated colleagues, collaboration with community partners and growth opportunities.Requirements:
Key Responsibilities:
Case management including an assessment of the needs of the child/adolescent and family Active collaboration with community providers and services Utilize therapeutic skills Collaboration with multidisciplinary teams Developing treatment plans, completing clinical notes and other documentation utilizing an electronic medical recordEducation:
Bachelor's degree in psychology, social work, or education/counseling from an accredited college or university.Job type:

Commitment to teamwork Self-Starter, Independent Time management and organizational skills are a must. Excellent communication ability with co-workers, staff, clients, and community partners.Qualifications:
