Full Job Description Details:

Now Hiring Crisis Case Managers, to work in a crisis stabilization setting in Cherry Hill, Camden County, NJ. The program's services are provided 24/7, 365 days per year. The Crisis Center works with individuals at their most vulnerable state and you will work with an interdisciplinary team to safely resolve behavioral health crisis through intervention, assessment and stabilization.

Schedule:

Day and Evening positions available! 8:30am to 5pm Sunday-Thursday 11:30am to 8pm Tuesday-Saturday 8:30am to 5pm

Tuesday-Saturday Responsibilities:

Provide intensive community-based services to consumers who has recently been discharged from Crisis Screening Center Facilitate service linkage in the mental health and non-mental health; health system; coordinating and integrating services from multiple providers Provide direct service support including: transportation, coordinating treatment team meeting; filing applications for services; Medicaid certs; advocacy; escort to appointment; daily living skill remediation and money management. Maintain a consumer clinical record (electronically) that accurately reflects all services provided on a timely basis and meet Agency.

Provide psychiatric emergency services including:

crisis assessment, intervention, outreach and screening for commitment to promote the safety of the client in crisis. Work collaboratively with the Clinical Team. Drive and Travel within Camden County. Other duties as needed Why Choose Oaks?

Qualifications:

Bachelor's degree in mental health related field with screener certification/ eligibility. A minimum of three (3) years' experience in mental health, at least one of which must be in a crisis related setting. Valid drivers license in good standing required.
