Job Description

Case Manager - Mental Health Diversion Program (MHDP) - Atlantic County Support individuals with mental illness as they navigate the justice system and work toward stability. As a Case Manager in the Mental Health Diversion Program, you'll provide community-based support to clients recently released from jail, at risk of incarceration, or involved with the courts. This program connects justice-involved individuals with behavioral health treatment, recovery supports, and essential community resources.
Key Responsibilities:
Provide community-based case management to individuals with mental health and justice involvement Assist with service planning and support clients in engaging in treatment and recovery services Conduct outreach and field visits to maintain client engagement and monitor progress Link clients to mental health, substance use, medical, housing, and vocational resources Communicate with probation, courts, and treatment providers to coordinate care Facilitate referrals to housing, employment, benefits, and wraparound supports Maintain timely and accurate documentation in the electronic health record (EHR) Participate in team meetings, supervision, and case conferences Uphold agency standards and confidentiality requirements
Position Requirements:
Bachelor's degree in Social Work, Psychology, Counseling, Criminal Justice, or related field Experience working with individuals with mental health and/or substance use needs preferred Experience with justice-involved populations strongly preferred Valid NJ Driver's License required At JFS, you'll be part of a passionate, diverse team that's been making a real difference in the community for nearly 100 years.
